Wound Care Case Manager

Full Time

Job description

Wound Care Case Manager (Remote)

If you’re the kind of Case Manager who lights up when guiding patients through challenging wound journeys, someone who leads with empathy, clarity, and genuine care, you may be exactly who our client is looking for. They are expanding their team with professionals who bring heart, curiosity, professionalism, and a true passion for patient wellbeing.

Every interaction matters. Their work centers around one mission: helping people heal. We meet patients where they are, support them through complex or chronic wound concerns, and make sure they feel seen, heard, and supported every step of the way. In this role, you become the steady presence connecting patients, families, and clinical teams ensuring their care feels coordinated, informed, and deeply human.

You’ll be stepping into a culture that celebrates the way we work together just as much as the work we do. Their Core Values, Teamwork, Resilience, Open-Mindedness, Ownership, and Positivity, guide how we show up for our patients and for each other. If those values resonate with you, and you’re excited to bring compassion and clarity to those who need it most, you’ll find a meaningful home with our client.

  • Have strong experience with EMR/case-management software, a good grasp of healthcare and insurance systems, and the ability to work effectively with multidisciplinary teams.
  • Strong communication skills, excellent organization, and the ability to keep things running smoothly.
  • A patient-first mindset, driven by integrity, empathy, and problem-solving.
