Social Worker for Youth Center - Sept-Îles in Cote Nord

We are looking for Social Workers with proven experience in youth center and/or DPJ to join our team in Sept-Iles in Cote nord Mission Principal : - Welcome and evaluate the needs of individuals and families and plan the appropriate intervention - Participate in the setting up and coordination of social services and programs - Collaborate with other health professionals to ensure a multidisciplinary approach - Provide psychological support and guidance to individuals and families - Develop and implement individualized plans and interventions - Participate in the development and implementation of policies and procedures - Collaborate with other stakeholders to ensure the continuity of services - Participate in the evaluation and improvement of services - Develop and maintain relationships with community partners and stakeholders - Participate in the development and implementation of training programs - Participate in the development and implementation of research projects - Participate in the development and implementation of evaluation and improvement projects - Fournir un soutien psychosocial et guidage aux individus et aux familles - Développer et mettre en œuvre des plans et interventions individualisés - Participer au développement et à la mise en œuvre de politiques et de procédures - Collaborer avec d'autres intervenants pour assurer la continuité des services - Participer à l'évaluation et à l'amélioration des services - Développer et maintenir des relations avec les partenaires communautaires et les intervenants - Participer au développement et à la mise en œuvre de programmes de formation - Participer au développement et à la mise en œuvre de projets de recherche - Participer au développement et à la mise en œuvre de projets d'évaluation et d'amélioration
