YOUR TASKS: Individual care: creation of therapy plans and psychosocial counseling including crisis intervention and companion support. All-day support: help with integration and preservation of the independence of residents. Network & Cooperation: close cooperation with home and nursing service management, administration, and participation in team and service meetings. Counseling & Communication: qualified information on social benefits, correspondence with authorities and social institutions. Engagement & Development: support for acquiring new customers to increase occupancy rates as well as gaining and accompanying honorary visitation services. YOUR PROFILE: Qualification: state-recognized social worker/social worker (m/w/d) or comparable training. Professional knowledge: good knowledge of social law, especially SGB XI, XII, and V. Experience & Technology: bed management and safe handling of common IT systems. Personality: empathy, social competence, and joy in working with older, care-dependent people.
