The Medical-Social Centers have been commissioned to help people of all ages who are dependent or have health problems, as long as possible in their living environment and support the relatives. The ASPMAD, with its 9 CMS, cares for customers at home and serves 105 communities in the North. We are looking for a social worker for our CMS X Vallée de Joux. Start: July 1st. Your tasks: As a social worker, your role includes advanced support for people of all ages who are dependent or have health problems, as long as possible in their living environment and support the relatives. This includes providing information and advice on healthcare, social services, and other topics relevant to daily life. You will also be responsible for coordinating care and supporting the relatives of those affected. Your tasks will also include advanced support for people with mental health issues, substance abuse, or other challenges. You will work closely with other professionals in the field, such as doctors, therapists, and social workers, to provide comprehensive support to our customers. Your tasks will be carried out in a team-oriented environment and require strong communication skills, empathy, and a willingness to learn and adapt.
