Social Worker (Permanent Half-Day) - Mossel Bay

Nurture Health is seeking a compassionate, accountable and skilled permanent half-day Social Worker to support patients and their families throughout their physical rehabilitation journey.

Reporting to the Therapy Manager, you will identify psychosocial risks and barriers, provide targeted interventions and work closely with the interdisciplinary team to support patients towards a safe and sustainable discharge.

Key responsibilities

• Holistically assess patients psychosocial needs, develop and provide appropriate interventions, including counselling and grief support, and maintain accurate psychosocial documentation.
• Facilitate family training and meetings, support community reintegration and return-to-work planning, and liaise with relevant workplace, community and support structures.
• Collaborate with the interdisciplinary team to communicate psychosocial risks, goals and progress, contribute to informed care decisions, and plan safe and appropriate patient discharge.

Requirements

• Registered with the Professional Board for Social Work of the South African Council for Social Service Professions (SACSSP) in the category of Social Worker.
• Proven experience developing and delivering social work interventions for medically complex and/or rehabilitation patients, with sound knowledge of the Social Service Professions Act, professional guidelines and ethical requirements.
• A passion for empowerment and physical rehabilitation, with strong empathy, accountability, teamwork and adaptability, together with the ability to establish and maintain appropriate professional boundaries.
