Job Title: Social Worker - Mental Health Acute Discharge Location: Stoke Pay Rate: £38.14 per hour Job Description: Liquid Personnel is recruiting a Social Worker to join its client’s Adults Mental Health Acute Discharge team based in Stoke. As a Mental Health Social Worker involved in Section 117 aftercare discharges, your primary responsibility is to ensure that individuals who have been detained under certain sections of the Mental Health Act (typically Sections 3, 37, 47, or 48) receive appropriate support and care following their discharge from hospital. This will involve working closely with the individual, their family/carers, and other relevant professionals to develop a personalized aftercare plan that meets their needs and promotes their recovery. You will also be responsible for ensuring that all necessary documentation is completed accurately and in a timely manner. The ideal candidate will have experience of working in mental health settings, particularly in acute discharge roles, and possess excellent communication and interpersonal skills.
