About The Role

Our single goal is provide a service that builds 'healthier rural people and thriving communities', and in this role, you’ll grow long-term relationships with patients, simplifying referral pathways and access to care to help them manage their health within their own communities.

This is a rewarding role where you can be involved in people’s lives, not just their illnesses.

As an important part of the multidisciplinary care team, you’ll deliver nursing care through a flexible service delivery model, both in community and ambulatory settings and will lead, direct and supervise other care roles in the team, including nurses, students and Aboriginal Health Workers (AHW) to support the delivery of quality care.

The successful applicant will demonstrate knowledge and experience in chronic disease prevention, health improvement initiatives and other evidence based hospitalisation prevention strategies to assist in the overall health and wellbeing of the community of Western NSW.
