You’ll be the key contact for clients recovering from injury or facing challenges, helping them return to independence through tailored support and coordination. You’ll work 1:1 with clients, collaborating with professionals across health, social services, and community sectors. Your expertise will ensure seamless transitions between healthcare providers, rehabilitation services, and other relevant stakeholders. This role is all about building strong relationships, communicating effectively, and empowering individuals to achieve their goals.
