This isn’t your average aged care physio role.
Whether you’re experienced in aged care or have been curious to step into the sector, this role offers real diversity; blending a community-style approach within an aged care home. You’ll support clients to build strength, mobility and confidence so they can live their most independent, joyful lives across aged care, home care and disability settings. No two days look the same, and you’ll have the freedom to be creative, backed by a supportive team that keeps everything running smoothly.
You’ll deliver three structured components of the Physiotherapy service for our aged care partner in this role:

You’ll deliver three structured components of the Physiotherapy service for our aged care partner in this role:
- Assessment and Care Planning
Complete core clinical tasks such as assessments, reviews, referrals, meetings (HOD/MDT/case conferences) and staff education, as part of the multidisciplinary team on site. - Deliver Manual Handling Training
Our on-site training focuses on creating a practical, hands-on and engaging experience for where participants are challenged to complete tasks and demonstrate their knowledge of manual handling. - Wellness and Reablement Programs
Flexible hours to deliver individually tailored wellness and reablement programs aligned with each resident’s goals and needs. Our innovative suite of wellness and reablement programs (funded under HELF) have been pre-designed by our expert Clinical Team, it will be your role to identify potential participants, enhance uptake, and work with the on-site and Guide’s clinical and admissions team to build a comprehensive offering within your home.

- Assess, plan and deliver evidence-based, goal orientated, and outcome-driven therapy
- Build a vibrant wellness culture that enhances resident outcomes, and engagement.
- Deliver Wellness and Reablement Programs within HELF services
- Prescribe & document exercise programs for residents - Work with an onsite Strength & Conditioning (S&C) team to optimise resident outcomes
- Build strong client relationships
- Deliver therapy though 1:1's and group programs
- Document clearly and efficiently
- Collaborate with S&C, clinical & multidisciplinary teams
- Report and be accountable to HELF service KPIs
- AHPRA registration
- Police Check
- NDIS Worker Check + Orientation
- COVID-19 & Flu vaccination
- Full working rights
- Private Practice Experience desirable
- Home Care experience desirable
