Physiotherapist – Aged Care & Community Outreach Part-Time | Residential Setting + Home Visits | East Fremantle
We are seeking a part-time Physiotherapist to join a dedicated Allied Health team based in East Fremantle. This role is primarily based in a residential aged care setting and also includes providing outreach physiotherapy services to clients in their homes. The position focuses on delivering high-quality, person-centred care to support independence, mobility, and quality of life.
About the Role You will:

We are seeking a part-time Physiotherapist to join a dedicated Allied Health team based in East Fremantle. This role is primarily based in a residential aged care setting and also includes providing outreach physiotherapy services to clients in their homes. The position focuses on delivering high-quality, person-centred care to support independence, mobility, and quality of life.
About the Role You will:
- Deliver high-quality physiotherapy services to residents and community clients
- Complete comprehensive assessments, care plans, and clinical recommendations
- Provide outreach physiotherapy services in clients’ homes
- Support and maintain an effective falls prevention and management program
- Build strong rapport with residents and clients
- Work collaboratively with multidisciplinary care teams to enhance wellbeing
- Complement hospital outpatient programs and services such as RITH to ensure continuity of care
- Contribute to quality improvement initiatives across allied health and residential services
- Comply with allied health standards, clinical governance, and Workplace Health and Safety policies
- Based in East Fremantle
- Residential aged care facility with community outreach component
- AHPRA registration as a Physiotherapist
- A minimum of 3 years’ physiotherapy experience
- A strong commitment to person-centred care
- The ability to work independently and as part of a multidisciplinary team
- Excellent communication and clinical reasoning skills
- A genuine passion for improving the lives of older adults
