As a Pflegesatzverhandler (m/w/d), you will be responsible for: • Developing and implementing care packages tailored to individual needs, • Collaborating with healthcare professionals and other stakeholders to ensure effective care delivery, • Maintaining accurate records of patient data and treatment plans, • Participating in quality control measures to continuously improve care processes. Requirements: • Bachelor's or Master's degree in a relevant field (e.g., healthcare, social sciences), • 2-3 years of experience in care coordination or a related field, • Excellent communication and interpersonal skills, • Ability to work independently and as part of a team, • Proficiency in MS Office applications.
