As a Care Professional (m/w/d), you will be part of our multidisciplinary team that provides comprehensive care to older people in need. Your tasks will include: • Providing individualized care and support to older people with various needs and abilities • Developing and implementing personalized care plans in collaboration with the interdisciplinary team • Maintaining accurate records and documentation of patient care and treatment • Participating in staff meetings and training sessions to stay up-to-date on best practices and new developments in the field • Collaborating with other healthcare professionals, family members, and caregivers to ensure seamless transitions and continuity of care.
