Your tasks will include: • Conducting gerontopsychiatric assessments and developing individualized care plans for our residents. • Providing emotional support and counseling to our residents, their families and caregivers. • Collaborating with other healthcare professionals to develop and implement comprehensive care concepts. • Participating in interdisciplinary teams to ensure seamless care coordination. • Documenting patient information accurately and maintaining confidentiality. • Adapting to changing circumstances and priorities in a fast-paced environment.
