Occupational Therapist - Hybrid Clinic and Community

About the Role: We are seeking an Occupational Therapist to take on a hybrid clinic and community-based role in Caboolture. Provide quality services to clients, both in the clinic and within communities, through assessment, treatment planning, implementation and evaluation of occupational therapy programs. This will involve working closely with multidisciplinary teams, including physiotherapists, speech therapists, psychologists and other healthcare professionals. The successful candidate will be expected to work independently and as part of a team, using their skills and expertise to deliver high-quality services that meet the needs of clients. Key responsibilities include: • Conducting initial assessments and developing treatment plans for clients with various physical, cognitive and emotional disabilities. • Implementing and evaluating occupational therapy programs in both clinic and community settings. • Collaborating with multidisciplinary teams to develop and implement comprehensive care plans for clients. • Providing education and support to clients, families and carers on occupational therapy strategies and techniques. • Maintaining accurate records of client interactions, assessments and treatment plans. • Participating in quality improvement initiatives and contributing to the development of clinical guidelines and policies. • Working collaboratively with other healthcare professionals to ensure seamless transitions between clinic and community settings.
