Our client is expanding their services and is on the lookout for an experienced Occupational Therapist to join their dedicated team in Melbourne's south east. Focusing on Home Care Package clients, they are committed to delivering high-quality care and making a significant impact on their elderly clients. You will conduct initial assessments for Home Care Package clients, evaluating their mobility and providing recommendations for assistive technology and home modifications. Clear and open communication is essential for building strong relationships with clients and their families. As an Occupational Therapist in this role, you will be responsible for: • Conducting comprehensive assessments of clients' physical and cognitive abilities to identify areas where they may require assistance or support. • Developing personalized care plans that address clients' unique needs and goals. • Providing education and training to clients and their families on how to use assistive technology and make home modifications to improve mobility and independence. • Collaborating with other healthcare professionals, such as physiotherapists, speech therapists, and social workers, to ensure comprehensive care for clients. • Maintaining accurate and detailed records of client assessments, treatment plans, and progress.
