Occupational Therapist – Aged Care (Home Care Packages)

Location: Bibra Lake, WA
Leave: 5 weeks annual leave

About the role
Join our team and actively support older adults to live safely and independently at home. You’ll manage an aged care caseload with a focus on Home Care Packages. You’ll:

Conduct in-home functional assessments and deliver interventions.
Develop and review personalised care plans.
Collaborate with clients, families and care teams to set and meet goals.
Recommend equipment, home modifications and strategies to enhance daily living.

What we’re looking for

Degree in Occupational Therapy and AHPRA registration.
Strong communication and time management skills.
Commitment to client-centred, evidence-based practice.
Graduates through to experienced clinicians welcome.
Aged care experience is not required – you must be motivated and driven to work in aged care.
