Occupational Therapist - Clinic and Community Rehabilitation

Our Client: Our client is a growing national provider of in-home care, nursing, and allied health services for people with disabilities. They work with people across the lifespan, providing intervention within their rehabilitation clinic, supported accommodation facility and within the individual's home, school, or community. About the Role: We are seeking an Occupational Therapist to take an NDIS/LSA caseload working in a multidisciplinary team who are passionate about working collaboratively with individuals, families, and carers to achieve optimal health outcomes. The successful candidate will be responsible for assessing, planning, implementing, and evaluating occupational therapy interventions that promote independence, participation, and community reintegration. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. Key responsibilities include: • Conducting comprehensive assessments of individuals with disabilities to identify their strengths, needs, and goals. • Developing and implementing individualized occupational therapy plans that promote independence, participation, and community reintegration. • Providing education and support to individuals, families, and carers on strategies for promoting independence, participation, and community reintegration. • Collaborating with multidisciplinary teams to ensure comprehensive care and optimal health outcomes. • Maintaining accurate records of assessments, interventions, and outcomes. • Participating in professional development opportunities to stay current with best practices and evidence-based research.
