About the role The Nurse Practitioner will be responsible for providing high-quality, patient-centered care to whānau, and will work closely with our multidisciplinary team to deliver comprehensive health services. This role is an exciting opportunity to join a dynamic and innovative organization that is committed to improving the health and wellbeing of whānau. Key responsibilities will include: • Providing primary care to whānau, including health assessments, diagnosis, and treatment of common medical conditions • Developing and implementing individualized care plans that take into account the unique needs and circumstances of each whānau member • Collaborating with other healthcare professionals to provide comprehensive care to whānau • Participating in quality improvement initiatives to ensure the delivery of high-quality, patient-centered care • Maintaining accurate and up-to-date records of patient interactions and care provided • Participating in professional development opportunities to stay current with best practices in primary care • Working collaboratively with other team members to achieve shared goals and objectives.
