Responsibilities: • Provide high-quality, compassionate care to seriously ill patients with complex medical and social needs. • Conduct thorough assessments, develop individualized care plans, and coordinate care with other healthcare professionals. • Collaborate with interdisciplinary teams to provide comprehensive care that addresses the physical, emotional, and spiritual needs of our patients. • Educate patients and their families about disease processes, treatment options, and self-care strategies. • Facilitate advance care planning discussions with patients and their families to ensure that their goals and values are respected. • Participate in quality improvement initiatives to improve patient outcomes and satisfaction. Requirements: • Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP). • Current certification as a Nurse Practitioner (NP) in the state where you will be practicing. • Minimum 2 years of experience working with seriously ill patients, preferably in hospice and palliative care. • Strong communication and interpersonal skills to work effectively with patients, families, and healthcare professionals. • Ability to work independently and as part of a team. • Familiarity with electronic medical records (EMRs) and other healthcare technology.
