Nurse for our mobile palliative care team (m/f/x)

As a member of the mobile palliative care team, you will be assigned to various missions:

• Clinical support and accompaniment
  • Ensure comprehensive patient care: pain management, symptom control, and palliative care
  • Evaluate patients' physical, psychological, social, and spiritual needs and those of their loved ones
  • Offer emotional, psychosocial, and spiritual support adapted to the patient's convictions
  • P prepare and organize continuity of care at home or in an adapted structure, in collaboration with treating physicians, social services, and home palliative care teams
  • Facilitate ethical discussions and accompany end-of-life decisions (palliative sedation, treatment withdrawal, euthanasia…), respecting each individual's philosophical and religious beliefs
  • Inform and guide patients in writing and updating their advance directives
  • Offer psychological support and bereavement accompaniment to loved ones
    
• Interdisciplinary collaboration
  • Work closely with hospital care teams, treating physicians, paramedical services, and external partners
  • Participate in multidisciplinary meetings and complex case consultations
  • Counsel and support frontline teams in palliative care
    
• Training, sensitization, and quality
  • Contribute to ongoing training for caregivers on palliative care topics
  • Promote a palliative culture within the institution
  • Participate in developing, updating, and implementing specific protocols and procedures
  • Participate in evaluating and continuously improving palliative care practices
  • Contribute to institutional projects aimed at optimizing quality and coordination of care
  • Ensure statistical and qualitative follow-up of mobile palliative care team interventions (annual report etc…)
