In collaboration with the medical team, you will be responsible for ensuring coordination of care and global patient follow-up. - Coordinate care and follow-up between different actors involved in patient care - Ensure partnership with different sectors and social workers - Evaluate troubles, environment, resources, and patient autonomy degree - Propose and encourage alternative activities based on capacities and interests - Realize educational care and monitor treatment adherence
