Medical Coder

- Assigns ICD-10 and/or CPT/HCPCS codes, as appropriate, and abstracts pertinent information from patient medical records. - Ensures optimal reimbursement of all cases in compliance with CMS policies and procedures and Official Coding Guidelines. - Implements medical center’s physician query process when code assignments are not straightforward or documentation in the medical record is inadequate, ambiguous or unclear for coding purposes. - Maintains updated knowledge of coding guidelines and regularly reviews and updates coding practices to ensure compliance with regulatory requirements. - Collaborates with physicians and other healthcare professionals to clarify any ambiguities or inconsistencies in patient records. - Utilizes computerized coding systems and software applications to facilitate the coding process. - Participates in quality assurance activities to ensure accurate and complete coding of medical records. - Maintains confidentiality and adheres to HIPAA guidelines when handling patient information.
