We offer permanent contracts, liberal installation, and fixed-term contracts. You join a multidisciplinary team composed of 3 other practitioners, a nursing manager, nurses, caregivers, a psychologist, a social worker, a dietician, an occupational therapist who work in cooperation with healthcare professionals (IDEL, general practitioners and specialists…).

Ready to take on new challenges as a Home Care Physician coordinator (F/H) at a recognized establishment?

Main responsibilities :
- Coordinate patient care at home in collaboration with the care team.
- Communicate effectively with various healthcare network actors involved.
- Ensure medical expertise for the multidisciplinary team, as well as for general practitioners within the framework of patient care management.
- Guarantee optimal patient care by working closely with other health professionals.
- Follow up on patient health and adapt their care plan accordingly in collaboration with the service's female manager
Participate in continuity of care: 2nd-line medical telephonic duties, one week out of six from Monday to Friday, and every two months

- Company vehicle We are looking for a doctor (general practitioner, emergency physician, geriatrician...) f/h registered with the order, interested in hospitalization at home and care coordination, with an appetite for working in a multidisciplinary team.

- Experience in palliative care, or wounds and scarring, or HAD would be appreciated but is not mandatory
- Knowledge of PMSI would be appreciated
