The SSR takes charge of patients requiring medical follow-up and rehabilitation care after hospitalization, with a clear objective: to facilitate return home and restore patient autonomy.
Your missions
- Ensure medical care for SSR patients.
- Actively participate in patient visits and develop personalized care plans in collaboration with the multidisciplinary team.
- Transcribe and manage medical records using our modern computer system.
- Work closely with the coordinating doctor, physiotherapists, nurses, caregivers, and other healthcare professionals to ensure comprehensive care.
Contribute to multidisciplinary meetings to share your expertise and advance medical projects at the establishment.
Working conditions: Hours: Monday to Friday, 8:30 am-5:30 pm.
Obligations: To be defined according to needs.
Full-time or part-time CDI with 25 paid vacation days + RTT,
Attractive remuneration,
Benefits: Housing assistance in case of relocation, family integration support, and more! You must hold a >Bac+5 degree and have at least 1 year of experience.
You demonstrate:
- Ability to coordinate multidisciplinary teams,
- Ease of relationship building,
- Initiative strength,
- Knowledge and/or interest in the medico-social sector and the received population,
- Computer knowledge.
