Your main missions :
* Medical evaluation and validation of admissions in HAD
* Coordination of care between hospital, general medicine and liberal interveners
* Home visits and development of personalized care projects
* Follow-up on medical files and link with the treating doctor
* Participation in transmissions, synthesis meetings and transversal projects
* Contribution to team training and practice improvement
* Support for the development of telemedicine and innovations in HAD
Partnerships :
Collaboration with the Lunéville CH and partner structures
Participation in certification, promotion of HAD and protocol writing
Join an engaged team for global, human and coordinated care at home. Profile: Holder of a Doctorate in Medicine degree, you are registered with the National Council of the Order of Physicians or have a European diploma allowing rapid registration with CNOM. Invested in your function, you are dynamic and able to work as a team.
