As a Medical Coordonnator (H/F), you are responsible for coordinating care and play a medical expert role with the management and teams. The position is a part-time CDI (0.50 ETP).
Your main missions include :
Developing and implementing the overall care project for the establishment.
Analyzing admission requests to ensure their compatibility with the institution's capacities.
Evaluating residents' care needs via AGGIR and PATHOS grids.
Coordinating resident care with the nurse coordinator, management, and external interveners.
Actively participating in quality approach and audits.
Animating a training program for caregiving teams.
Developing relationships with local gerontological networks.

Conditions for the position:
- Contract: CDI, senior staff, 0.5 to 0.6 ETP

- Salary: according to profile and experience


Profile sought :
Holder of a State Doctorate in Medicine diploma, with registration at the National Order of Physicians.
A geriatric or medical coordinator training is required (or in progress).
6-10 years' experience on a similar position is requested.
A driving license is essential, as the site is not served by public transportation.
