Coordinating Physician - Beaurevoir 02 M/F

The Coordinating Physician will ensure the coordination of resident care and will be involved in institutional life as a medical expert and advisor to the teams and management. They will carry out their missions in compliance with the code of ethics and gerontology recommendations, according to the texts describing the missions of a coordinating physician in an EHPAD. In collaboration with the healthcare team and management, your main missions will be: - To elaborate and supervise the implementation, with the assistance of the healthcare team, of the general care project, integrating into the residence's establishment project - To analyze medical admission requests, paying particular attention to the compatibility of their health status with the institution's care capabilities - To evaluate and validate the level of independence (AGGIR scales), the required care needs (PATHOS), and the standardized geriatric assessment in order to evaluate the risks inherent in their pathologies - To lead the staff training program - To ensure and organize the smooth flow of oral and written communication, notably by leading the weekly multidisciplinary meeting and facilitating coordination with external practitioners - To coordinate care jointly with paramedical practitioners, the Coordinating Nurse, and Management - To organize and lead meetings with residents' relatives - To participate in professional practice evaluations, Quality audits, and the quality and certification process - To engage in a local gerontology network in liaison with healthcare facilities and health professionals in the area, notably the Geriatric Coordination Commission, agreements, and the development of innovative regional projects #LI-TT1
