You coordinate patient pathways and therapeutic projects with teams, healthcare professionals such as liberal doctors, psychologists, psychiatrists, and other specialties, families, and health partners in the territory, particularly medical assistants for accompanied people.

Main missions:
Realize an initial diagnosis and evaluation, then a re-evaluation whenever necessary, by:
- performing a consultation and possible orientation towards the most suitable structure,
- searching for antecedents and pathologies,
- visiting the place of life,
- contacting and ensuring relational follow-up with families, caregivers or curators.

Realize acts of screening and prevention by:
- performing an evaluation of motor functions, cognitive functions, psychic functions, sensory balances, and dental exams,
- evaluating autonomy and pathologies (GIR, PATHOS...),
- observing and monitoring the hygiene and nutrition of users,
- preventing and informing users about addictions (tobacco-alcohol-stuporants, games, screens...),
- informing caregivers.

Coordinate medical and paramedical care by:
- realizing follow-up on nurse, local or liberal paramedics' work,
- participating in transmissions between healthcare professionals and paramedics,
- performing interviews with liberal doctors, psychiatrists, and other specialists,
- establishing partnerships with hospital structures, mobile psychiatric follow-up teams, nursing protocols, HAD interventions, etc.,
- participating and setting up technologies like telemedicine, secure medication distribution, data transmission improvements, DMP...,
- integrating the care project into the user's personalized project and ensuring its implementation.

Part-time CDI 0.5 ETP (17.5h) according to the following allocation: 0.2 FAM + 0.3 SAMSAH

Possible work schedule arrangement -> planning on 1 or 2 days + interventions on site and/or by telemedicine. Doctor of Medicine degree registered with the Order, would be a plus if holder of a Certificate in Physical and Rehabilitation Medicine (MPR); sensitivity to the disability sector.

Appetite for teamwork in a multidisciplinary team within an institutional dynamic; autonomy, dynamism, and initiative spirit;
knowledge of public policies in the medical-social sector desired; knowledge of partners in the social, health, and medico-social sectors desired; mastery of digital tools desired.
