YOUR MISSION: Attached to the Direction and the Chief Physician of the service, you ensure the coordination and quality of the rehabilitation pathway. Your role revolves around three major axes:

Clinical Evaluation and Expertise :
- Perform functional evaluations of patients at home and define individualized therapeutic projects.
- Ensure the relevance and feasibility of rehabilitation objectives.
- Counsel prescribing doctors on adapting care protocols.

Coordination and Pilotage :
- Organize and coordinate interventions from liberal and salaried professionals (physical therapists, occupational therapists, speech therapists).
- Ensure constant communication between the city, hospital, and home to streamline patient pathways.
- Chair multidisciplinary synthesis meetings.

Logistical and Material Management :
- Evaluate needs for home adaptation and secure the patient's environment.
- Coordinate the placement and follow-up of medical equipment with external providers

- Salary: €1,614.38 to €3,000.00 per month depending on the duration of work and experience. Holder of the State Diploma in Massage-Therapy + current registration with the Order of Kinés.
Clinical experience desired and appetite for working in a multidisciplinary team.
Organizational capacity, rigor, and ease of relationship.
Driving license required (daily travel to the intervention zone).
