Managed Support Navigator

As a Navigator in our Managed Support team, you will provide outstanding service to all stakeholders involved, to ensure that our clients receive the best possible support. Your organisational skills and person-centered approach will mean that support plans are actioned in a timely manner, allowing our clients to have seamless access to a wide range of services and providers.

About the role …

  • Allocate workflow, assessing any potential risks or considerations, in accordance with NASC guidelines.
  • Provide crisis response to ensure immediate risk mitigation and comprehensive support for an immediate short-term response.
  • Respond with urgency to queries and risks ensuring effective solutions or escalations as required.
  • Conduct needs assessments, plan and coordinate support for clients with urgent needs.
  • Liaise with clinical teams, specialist services and coordinate comprehensive handovers to case managers for ongoing engagement and long-term planning.
  • Complete reviews for clients when needed, as well as peer reviews and recommendations for the team.
  • Work collaboratively to foster effective communication within the team, along with clear and accurate flow of documentation and information.

About you …

  • Proven experience in coordinating natural and formal supports and services for people with a range of physical, intellectual and sensory needs.
  • A passionate team player who is committed to Kaikaranga values
  • Exceptional communication and stakeholder relationship management skills, with the ability to relate to a diverse range of people
  • Highly organised with a keen eye for detail and the ability to maintain efficiency and a positive attitude when juggling a busy day.
  • Working knowledge of Te Tiriti o Waitangi and able to coach others in its practical, day to day application.
