To fill a position in the medical sector, we put our expertise in healthcare at your service. As part of the HAD team, you are a key player in patient care coordination. In this role, you:

• Organize hospitalization at home by conducting pre-admission evaluations (needs, social situation, necessary materials, etc.).
• Represent HAD to hospital prescribers, city doctors, and medical-social establishments.
• Participate in building and transmitting the admission file.
• Ensure liaison between different healthcare professionals (doctors, IDEL, partner structures, etc.).
• Contribute to promoting HAD's healthcare offer on the territory.
• Suggest and animate awareness or training actions.
• Collaborate in activity analysis and continuous improvement of practices.

  Diploma: State-certified Nurse

  
  

  Desired experience: Experience in HAD, care coordination, or with multidisciplinary teams

  
  

  Familiarity with hospital functioning, urban medicine, and medical-social partners

  
  

  Mastering computer tools (DPI, office software)

  
  

  Expected qualities: organization, autonomy, adaptability, ease of communication, pedagogy, team spirit.
