This is a Intensive Education Case Manager - Far North Queensland Region - Cairns Office role with Queensland Government based in Cairns, QLD, AU Queensland Government Role Seniority - mid level More about the Intensive Education Case Manager - Far North Queensland Region - Cairns Office role at Queensland Government The Intensive Education Case Manager is based in the Student Child and Family Connect Directorate and provides an opportunity to proactively support some of our most vulnerable students and families. The Intensive Education Case Manager will work within the Far North Queensland Region, providing a holistic case management service to students and their families experiencing complex needs impacting on their ability to engage in education and attend school. The Intensive Education Case Manager will: * Provide case management services to students and families in accordance with the department's policies and procedures. * Work in partnership with students, families, schools, and other agencies to develop and implement individualised support plans. * Monitor the progress of students and families, and adjust support plans as needed. * Maintain accurate records and case notes. * Participate in team meetings and professional development activities. Key responsibilities: * Undertake case management, including: * Conducting comprehensive needs assessments. * Developing and implementing individualised support plans. * Providing advocacy and support to students and families. * Monitoring student progress and adjusting support plans as needed. * Maintaining accurate records and case notes. * Liaise and work in partnership with schools, families and other agencies to enhance student outcomes. * Contribute to the development and implementation of regional strategies to improve student outcomes. * Participate in team meetings and professional development activities. About You: To be successful in this role, you will have: * A relevant tertiary qualification (e.g. social work, psychology, education) and/or demonstrated experience in case management. * Demonstrated experience in working with children and families experiencing complex needs. * Excellent communication and interpersonal skills. * Ability to work independently and as part of a team. * Knowledge of child protection and relevant legislation. * A current driver's license.
