The Intensive Education Case Manager will work within the Far North Queensland Region, providing a holistic case management service to students and their families experiencing complex needs impacting on their ability to engage in education and attend school. The Intensive Education Case Manager will: * Provide case management services to students and families in accordance with the department's policies and procedures. * Work in partnership with students, families, schools, and other agencies to develop and implement individualised support plans. * Monitor the progress of students and families, and adjust support plans as needed. * Maintain accurate records and case notes. * Participate in team meetings and professional development activities. Key responsibilities: * Undertake case management, including: * Conducting comprehensive needs assessments. * Developing and implementing individualised support plans. * Providing advocacy and support to students and families. * Monitoring student progress and adjusting support plans as needed. * Maintaining accurate records and case notes. * Liaise and work in partnership with schools, families and other agencies to enhance student outcomes. * Contribute to the development and implementation of regional strategies to improve student outcomes. * Participate in team meetings and professional development activities. About You: To be successful in this role, you will have: * A relevant tertiary qualification (e.g. social work, psychology, education) and/or demonstrated experience in case management. * Demonstrated experience in working with children and families experiencing complex needs. * Excellent communication and interpersonal skills. * Ability to work independently and as part of a team. * Knowledge of child protection and relevant legislation. * A current driver's license.
