Nursing Referral Officer in Home Care (CMS)

Job Description Mandated by CMS (Home Care) of the canton of Vaud, we are looking for Nursing Referral Officers M/F to join our team of dedicated home care poolists. Assess the patient's health status at home (clinical, autonomy, environment) and Develop a personalized care plan tailored to their needs, taking into account their clinical history, functional abilities, and social context. Collaborate with healthcare professionals and other stakeholders to ensure continuity of care and optimal patient outcomes. Provide emotional support and guidance to patients and their families, promoting independence and self-care. Stay up-to-date with the latest developments in home care and participate in training and professional development opportunities to enhance your skills and knowledge.
