You will be responsible for coordinating paramedical care at home, ensuring the well-being of patients and various organizations of teams.
- Clinical support at a distance: Counsel and guide field teams (nursing assistants, IDE) on complex situations or deteriorations of health status.
- Interdisciplinarity: Link different professions (paramedical, social, medical) to ensure each intervention is complementary.
- Securing return home: Use your experience in SSR or Medicine to anticipate the risks of rehospitalization and adapt the aid plan immediately.
- Information management: Centralize health data and ensure information circulates in real-time between all actors in the care pathway.
