Within our establishment, you have the following missions :
- Patient care at home
- Participation in multidisciplinary meetings (Med Coo, IDEC, Psy, Assistant social, IDE, AS, medical secretary)
- Implementing the patient's bedside care protocol and communicating clinical elements useful for patient care through the care file.
- Participating in developing protocols and reevaluating them.
- Actively participating in the structure's project (quality/project establishment)
- Taking charge of patients, under the responsibility of the nurse coordinator, in their entirety and personalized manner alone or in collaboration with the Home Health Aide

The future collaborator must also demonstrate good qualities in :
- Active participation in any work meeting concerning the service and getting involved in continuous improvement.

- Adaptation, rigor, and organization.
- Discretion, respect, and punctuality.

Our client's proposal for this position is as follows :
- To be filled as soon as possible
- Part-time CDI (60%)
- Schedules in 7:30 hours
- RTT (12/year) + Fériés recovery (worked and non-worked)
- CSE benefits (gift cards, vacation checks, travel)
- Remuneration starting at €2770/month, depending on profile and experience
- Service vehicle provided
- Obligations according to predefined schedule We are looking for a Nurse (F/H) for a full-time CDI position in hospitalization-at-home, requiring one year of experience.
- State nursing diploma required
- Experience in coordination or HAD desired
- Mandatory B permit for departmental travel (service vehicle provided)
- Rigor, sense of organization, team spirit, adaptability, and availability
