Your missions will be: Evaluate the health status of the people received and analyze the care situations, Design and define personalized care projects, Plan and provide care, and evaluate it, Implement treatments, Manage and update the care files of the people, Respect protocols, prescriptions, and periodic visits, Manage relationships with medical and paramedical teams, Participate actively in multidisciplinary team meetings, Communicate effectively with families, Be an integral part of the care network and play an active role, Ensure the management of the pharmacy and the supply of small medical equipment stocks.

Have a sense of fieldwork and anticipation in a complex social environment, master professional writing and computer tools, empathy, benevolence, and adaptability.
