Your main tasks will be: - planning and ensuring the execution of medical care according to prescriptions. Ensure continuity of care by monitoring patients' conditions, administering medications as needed, and providing emotional support. Collaborate with other healthcare professionals to develop personalized care plans. Participate in multidisciplinary team meetings to discuss patient care and treatment options. Maintain accurate records of patient care and treatment outcomes. Provide education and training to patients and their families on disease management and prevention. Develop and implement individualized care plans based on patients' needs and preferences. Ensure compliance with established policies, procedures, and standards for patient care. Participate in quality improvement initiatives to improve patient care and outcomes. Collaborate with other healthcare professionals to develop and implement evidence-based practices. Provide emotional support and comfort to patients and their families during times of crisis or stress. Develop and maintain relationships with patients, families, and caregivers to ensure continuity of care. Ensure that all aspects of patient care are carried out in a safe and respectful manner.
