Team Management: Animating, guiding and supporting the Home Care Assistant team on a daily basis.
Coordination of pathways: Ensuring coordination of care pathways in close liaison with the territory (liberal partners and medico-social structures).
Quality management: Guaranteeing the application of care protocols and the quality of services.
Individualized Care Project: Piloting the development and implementation of Individualized Care Projects (ICP), placing the user at the center of decisions.
Innovation: Actively contributing to the development of this innovative service platform. You must hold a Bachelor's degree + 3 years and have at least 6 months of experience.Diploma: Holder of the State Diploma in Nursing (IDE).
Experience: Significant and confirmed experience in team coordination and/or management in the medico-social or hospital sector is essential.
Qualities: Rigor, sense of organization, excellent relational qualities and listening skills, adaptability and a genuine appetite for the disability sector.
