Nurse Coordinator (F/H)

As a Nurse Coordinator, your days will be organized from 9:00 am to 5:00 pm.
Your main tasks will consist of evaluating new admissions to ensure optimal care, coordinating and following up on home care, and supervising healthcare teams. You will also be the essential point of contact with doctors, families, and all health partners. This position includes a night and weekend duty once a month. For this role, we are looking for a professional holder of the State Diploma in Nursing. Experience in Hospitalization at Home (HAD), care coordination, or technical home care would be a major asset.
You are an organized, autonomous, and rigorous person with excellent interpersonal skills. Mastery of computer tools and tracking of care is also essential.
Finally, a B permit is necessary for you to travel to patients' locations. Compensation will be established based on your experience and profile.
