Description of the post
- Ensure communication with VIVA member clients through the hotline telephone:
- Makes a medical triage if there is a medical problem
- Schedules appointments with family doctors, specialists, and/or therapists
- Answers their questions.
- Prepare client files by updating them according to their care journey
- Realize individual health plans in line with the medical file and clients' particularities within a global approach centered on prevention and case management of complex cases
- Meet with clients to personalize, adapt, and communicate about their health plan
- Ensure orientation, coordination, and follow-up of client care in collaboration with family doctors and the healthcare network
