Social workers / educators / psychologists or social scientists as case managers / case managers on a secondary and freelance basis.

Location: We are looking for employees across the states of North Rhine-Westphalia, Schleswig-Holstein, Mecklenburg-Western Pomerania, and Hamburg.

Do you already have experience and knowledge regarding the labor market, vocational knowledge, access requirements and pathways, as well as the content of further services for participation, as well as networking with regional structures of the labor market and vocational training?

Then you will surely enjoy acting as a "navigator" for people after medical or psychosomatic rehabilitation!

• Optimizing the clarification of medical care and the positive performance profile of rehabilitation patients
• Vocational counseling and job placement for rehabilitation patients: Profiling, application training, and job acquisition in pedagogical individual support with corresponding written documentation
• Person-centered "navigation function" for systematic care management
• Case-related cooperation within the network with payers and providers (rehabilitation facilities, social security institutions, counseling centers, self-help groups, etc.)

• Completed social sciences degree in social work, education, or psychology and at least 2 years of professional experience in the counseling field of vocational (re)orientation
• A completed degree in psychology additionally requires proof of at least 40 teaching units of social law knowledge.

• Vocational training in the health and social care sector or
• Rehabilitative further training, each with at least 5 years of professional experience in the counseling field of vocational (re)orientation
• each with proof of methodological and social law competencies (160 teaching units of counseling competence and 40 teaching units of social law), coupled with customer orientation, motivation, empathy, and persuasive power

Desirable:

• Further training in Case Management or CDMP qualification
• Knowledge of social security provider structures and rehabilitation law
• Experience in vocational and medical rehabilitation as well as in solution-oriented case management
• Good knowledge of MS Office programs (especially Word) and internet applications
• Mobility (near place of residence) – preferably with own car

• Activity on a self-employed basis, can be pursued part-time, and with attractive remuneration
