Founding Engineer Applied AI Engineering & Full-Stack (d/f/m)

Doctors spend up to 68% of their time on administration instead of focusing on their patients. Sikia AI builds KI agents that free doctors from administrative tasks, allowing them to concentrate on people rather than paperwork.

Our first product, a KI-assisted documentation and billing system, is already in use at clinics and practices across Germany, documenting thousands of patient conversations every week. The product works, doctors love it, and we're growing quickly. Now we're looking for our first Founding Engineer to take us to the next level.

Aufgaben

You'll build alongside our CTO and be responsible for central systems from start to finish. Your code will run in real clinics within weeks. You'll speak with doctors who use it, and then make it better. Maximum autonomy, true Founding equity (VSOP), a product from 0 to 1.

You're responsible for:

• The Python backend that drives our medical real-time documentation in production

• Live transcription of actual doctor-patient conversations: streaming audio, multiple STT providers, quality monitoring, and fallback mechanisms

• Our Applied AI core: LLM-evaluation pipelines, prompt optimization, multi-provider routing, and fallback chains, including cost and latency considerations for operating KI at scale

• Multi-cloud production infrastructure that keeps everything running reliably in clinical use

• Greenfield projects that define what our KI doctor will be next

Full-stack, if it counts: The work lies primarily in the backend and AI, but our product is based on Flutter (Windows and Web), and you'll jump in when needed. We're a small team and go where the problem is.

Qualifikation

You fit us if:

• You're really strong in Python, our core language, with at least three years of programming experience

• You build things and don't just work on tickets: You prototype on weekends, bring your own projects to life, and want to put your stamp on a product

• You've delivered real systems from start to finish and people rely on them

• You have high initiative: You recognize a problem, propose it, and deliver the solution

• You work with modern KI tools and want to dive deep into Applied AI (evaluation, prompting, routing, real-time pipelines), regardless of whether you're already experienced or eager to learn

• You think like a co-founder: You question our approach, recognize opportunities, and bring Sikia to the next level

• You empathize with doctors, patients, and patient families for whom we build

• You understand German on a conversational level

Vantage points:

• Dart and Flutter (especially Windows Desktop), a good product sense, a background in computer science, a MINT field or medicine, or you're an autodidact with serious delivered projects.
