About the Role We are seeking a compassionate and experienced Family Support Worker to join our growing team of professionals who share our passion for delivering exceptional support services. As a Family Support Worker, you will be responsible for providing person-centred support to individuals with disabilities and their families, focusing on building strong relationships, promoting independence, and empowering them to achieve their goals. This role is ideal for someone who is passionate about making a positive impact in the lives of others and values the importance of collaboration and communication. Key Responsibilities: • Provide person-centred support to individuals with disabilities and their families, focusing on building strong relationships and promoting independence. • Develop and implement individualised support plans that align with the NDIS goals and objectives. • Work collaboratively with other professionals, including therapists, doctors, and other healthcare providers, to ensure comprehensive care for our clients. • Provide emotional support and advocacy to individuals with disabilities and their families, empowering them to make informed decisions about their lives. • Maintain accurate records of client interactions, progress, and outcomes, ensuring compliance with NDIS reporting requirements. Requirements: • Bachelor's degree in a relevant field (e.g., social work, psychology, education) or equivalent experience. • Minimum 2 years' experience working with individuals with disabilities and their families. • Strong communication and interpersonal skills, with the ability to build strong relationships with clients and colleagues. • Ability to work independently and as part of a team, prioritising tasks and managing multiple responsibilities. • Understanding of NDIS principles, policies, and procedures, with experience working within the NDIS framework.
