In samenwerking met de andere zorgverleners neemt je deel aan de functionele revalidatie van intramurale patiënten. Daartoe moet je:
- De patiënt opnieuw de handelingen en activiteiten van het dagelijkse leven aanleren
- Deelnemen aan multidisciplinaire vergaderingen binnen de afdeling
- Regelmatig de nodige informatie uitwisselen met je collega's voor de follow-up van de patiënten
