Mission You accompany and support clients as well as their relatives in the implementation of specific and complex procedures with a therapeutic goal, by relying on assistance and support measures. You ensure contact establishment and coordination with relatives and the medical-social network. You are the main point of contact for the interdisciplinary team in social matters. You guarantee a reference and expertise function for members of the interdisciplinary team in social issues. You develop and maintain relationships with clients, relatives, and other stakeholders. You participate in the development and implementation of individualized support plans. You ensure the provision of information and advice to clients and their relatives. You are responsible for the coordination and organization of appointments and meetings. You ensure the maintenance of accurate and up-to-date records and documentation. You participate in the evaluation and monitoring of the effectiveness of the implemented measures. You contribute to the development and implementation of new procedures and methods. You maintain confidentiality and discretion regarding client information. You work independently and as part of a team, taking into account the needs and goals of clients and their relatives.
