Within a medical social establishment, your role will consist of guiding and supporting individuals with vulnerabilities
- Developing and implementing individualized projects tailored to the needs of accompanied individuals
- Collaborating closely with a multidisciplinary team to ensure coherent and appropriate follow-up
- Participating in continuous evaluation of progress and adjusting accompaniment strategies accordingly
