This position involves accompanying people with vulnerability to promote their autonomy and social integration.
- Develop and implement individualized socio-educational projects in collaboration with partners and families
- Ensure daily educational follow-up and intervene in crisis or conflict situations
- Coordinate actions with the multidisciplinary team to ensure the coherence of the user's care or life path
