Diabetes Educator (H/F/X)

  • Accompany, inform and educate the diabetic patient
    • During a first interview, review with the patient their medical history, information they have already received, daily habits, …
    • Create with the patient a personalized follow-up plan adapted to their needs. The topics covered are diverse :
    • The learning of self-control equipment use
    • Explanation of the disease and treatments
    • Counseling on lifestyle: diet, physical activity, tobacco, …
    • Practical tips for daily life: diabetes and travel, driving permit, …
    • Be attentive to the patient
    • Schedule education sessions during which the diabetes educator will ensure follow-up to support the patient, for example in case of treatment changes.
    • In collaboration with the patient, define achievable goals and monitor their realization (weight loss, quitting smoking, number of blood glucose tests, …)
  • Collaborate on the health monitoring of diabetic patients :
    • Examine the patient's self-monitoring logbook, collect objective parameters and analyze them (e.g. number of hyper- or hypoglycemic episodes)
    • Collect data from the patient's diabetes test results, observe their overall health status (e.g. weight variation, blood pressure, lab results, foot examination) and detect any changes;
    • Adjust treatment as needed after consulting with or under prescription from a doctor (e.g. adapting insulin type administered);
    • Schedule follow-up appointments for patients' medical tests.
  • Participate in the continuity and quality of care :
    • Inform the doctor on the patient's health status and consult with them about the treatment indicated;
    • Exchange information about patients with different healthcare providers (e.g. podiatrist, psychologist);
    • Attend multidisciplinary meetings and apply decisions made;
    • Contact external organizations to ensure patient follow-up.
  • Perform administrative tasks related to patients :
    • Create a file for the patient and keep it up-to-date (e.g. exams performed, medical equipment delivered) ;
    • Complete various formalities related to diabetes conventions or healthcare journeys and ensure follow-up.
  • Schedule management of medical supplies intended for patients :
    • Manage inventory (e.g. pumps, glucometers, information brochures) and medications (e.g. insulin): check stock levels and place orders according to internal procedures;
    • Select equipment based on the patient's needs and deliver it according to diabetes convention terms;
    • Receive suppliers, consult with them about existing equipment (e.g. defects, necessary replacements) or when introducing new equipment (e.g. interpreting user instructions).
  • Counsel and inform colleagues from various units to optimize care for diabetic patients :
    • Visit and evaluate diabetic patients treated in other healthcare units;
    • Share expertise as a specialized diabetes nurse with non-specialized nurses;
    • Support healthcare units in case of problems encountered with diabetic patients.
