Tasks - Planning, coordination, and implementation of the care process - Counseling and professional accompaniment of clients and their relatives - Conducting resource-oriented and goal-directed care measures - Coordination of interdisciplinary cooperation, e.g. with the Palliative Care Team - Active shaping of cooperation with all participating services, internally and externally Profile - Completed training as a Dipl.
