Our client, a renowned institution is seeking a profile of: Dietitian H/F Service rehabilitation, geriatrics Job start date: as soon as possible To develop personalized meal plans to promote recovery and well-being of patients, taking into account dietary restrictions and personal preferences Sensitize and educate patients and their families on the importance of proper nutrition and its impact on health and quality of life Provide guidance and support to healthcare professionals on the implementation of nutritional care plans Collaborate with other healthcare professionals to develop and implement a comprehensive care plan for patients Participate in multidisciplinary team meetings to discuss patient progress and adjust care plans as needed Develop and maintain relationships with patients, families, and healthcare professionals to ensure continuity of care Provide education and support to patients and their families on nutrition and health-related topics Conduct nutritional assessments and provide recommendations for patients with specific dietary needs or restrictions Identify and address potential barriers to healthy eating habits and develop strategies to promote healthy behaviors Participate in quality improvement initiatives to improve patient outcomes and satisfaction Develop and maintain knowledge of current research and best practices in the field of nutrition and rehabilitation
